The North Hollywood Integrated Care Clinic embodies the future of care delivery in Los Angeles County. Designed by HMC Architects in collaboration with Swinerton, this cutting-edge facility aims to revolutionize community health services by providing comprehensive care under one roof. Strategically located at 5300 West Tujunga Avenue, the three-story, 55,000-SF clinic will serve approximately 9,000 patients annually, becoming a beacon of hope and health for the community.

What Services Will Be Offered

Integrated Services for Mental and Public Health

Mental Health Services (DMH)

The North Hollywood Health Center will provide the following mental health services:

The new DMH clinic will provide comprehensive specialty mental health services to North Hollywood residents and the surrounding communities for individuals ages 18 and older with a serious and persistent mental illness.

The goal is to provide easy and timely access to mental health services to underserved and underrepresented community members from different cultural backgrounds to achieve mental and social wellbeing, personal recovery, and have the best possible health outcomes.

The services will include mental health services, targeted case management, rehabilitation, groups, crisis intervention, and medication supportive services. The vision is to implement an integrated care services model where patients can access various services across the healthcare spectrum for mental health, physical health, and public health services.

Our mission is to optimize the hope, well-being, and life trajectory of Los Angeles County’s most vulnerable through access to care and resources that promote not only independence and personal recovery but also connectedness and community reintegration.

Public Health Services (DPH)

The North Hollywood Health Center will provide the following low-cost and free health services:

  • Sexual Health Services
    • Family planning
    • STD testing and treatment
    • HIV testing
    • Mpox testing
  • Immunizations
    • Childhood
    • COVID-19
    • FLU
    • Mpox
  • Disease Outbreak Management – the facility will house nurses, physicians, and public health investigators to conduct field visits to manage disease outbreaks in the local area, including for measles, tuberculosis, and Mpox.
  • Wellness Community – the facility will provide a wellness community center where residents can take workshops and access services related to their health and wellbeing, including:
    • Fresh free produce distribution
    • Cooking/nutrition – Live demo kitchen
    • Car seat safety
    • Lactation support
    • Art therapy
    • Meditation
    • Exercise/dance
    • Health talks with a nurse
    • Substance abuse prevention
    • Systems navigation assistance
    • Emergency preparedness
    • Youth programming
  • Other support: Narcan, Fentanyl strips, COVID tests, TB test kits, condoms, menstrual/hygiene products

Department of Health Services (DHS)

The Department of Health Services mission is to advance the health of our patients and our communities by providing extraordinary care. 

The North Hollywood Health Center will provide adult primary care services on-site for empaneled patients.  We have a primary care medical home model where a team of staff including provider, nurse, social workers, and others work together for your health.  We are a health system with over 1,000 doctors across more than 25 locations to serve you.  With primary care comes access to many health care services in our network, including specialty care, inpatient care, emergency and urgent care, laboratory and radiology services, nurse advice line, pharmacy services, and more.
